Intensive Family Intervention Services

Overview

Intensive Family Intervention Services (IFIS) provides in-home services to children for 45 days to help them successfully reintegrate into their families, homes and schools after a period of 24-hour inpatient care, Partial Hospitalization Program (PHP) or Intensive Outpatient Program (IOP) treatment.

 

Program Details

When any household requires outside involvement, it helps to have all necessary resources available during the window of opportunity afforded for intervention.

That’s why our licensed therapists are available 24/7 for crisis intervention after providing an initial family assessment. Specialists are also in the home at least twice a week and make additional multiple phone contacts with the family weekly. Specialists are also available to the family 24/7 for intensive family support.

 

Family Assessment

A comprehensive family assessment is completed by a licensed therapist within 72 hours of discharge from a 24-hour inpatient setting. This assessment is created to ensure that the client and family have a full understanding of the discharge plan, to measure the family’s commitment to the discharge plan, to evaluate strengths and needs, and to create a Plan of Care, Crisis Plan, and a Safety Contract.

Intensive In-Home Parent Support and Education

Intensive Family Intervention Services specialists will work with the child or children’s family to review outpatient appointments and transportation plan, review medication orders and encourage compliance in the home, support parents/caregivers in implementing structure in the home, assist the family in gaining knowledge of behavioral health issues, educate the family to improve parenting, conflict resolution and problem-solving skills, provide ongoing monitoring for safety and stability in the home, and assist in problem-solving situations and upcoming challenges.

 

Service Coordination

Intensive Family Intervention Services specialists will provide assistance with educational advocacy, connect the child or children with a health plan case manager and coordinate care with all members’ providers.

Care Coordination

Intensive Family Intervention Services specialists will communicate with the health plan case manager on the progress of the child or children at least once a week. Specialist will submit progress notes to the health plan case manager to ensure ongoing need for current level of care, level of family participation and detailed information on the services being provided. IFIS specialist will update goals established on the plan of care each week.

 

24/7 Crisis Intervention

Master-level therapists will be available by phone 24 hours a day for crisis intervention services. The therapist will be available to come to the members’ homes to conduct a formal clinical assessment if necessary.

 

24/7 Intensive Home Support

Intensive Family Intervention Services specialist will be available by phone 24 hours a day to provide intensive home support. These services will be provided by phone or in person, as the situation demands. The specialist will be available to council and advise the client and/or family during a difficult situation. Families are encouraged to contact the specialist before a situation worsens to the point that there may be a safety issue.

 

Weekly Rounds

Great Circle staff meet weekly with the health plan case manager and other health plan clinical professionals to review the child or children’s goals and progress, to make alterations to the plan or care goals, and to update discharge plans as needed.
